Enter Publix Store Customer Satisfaction Survey

If you visited a Publix store and made a purchase recently, you might received a cash register receipt with a unique 19-digit code that can be used to take part in the Publix Customer Satisfaction survey and enter their Monthly Sweepstakes for a chance to win a $1,000 Kmart gift card.

How to enter & win:

To begin the survey, go to the website www.publixsurvey.com, then select your favorite language to continue.

In the next step, you will be asked to enter your 19-digit code found on your receipt. Enter it correctly and click the "Next" button to continue.

Then you need to answer all the survey questions regarding your shopping experience. It will take you approximately 5 to 10 minutes to complete.

At the end of the survey, you will be automatically entered into the sweepstakes.

Limit three entries per person/e-mail address. You must be a legal resident of the United States who are 18 or older to enter & win.
